1. Client Responsibility Commitment to Treatment Plan: Achieving optimal results requires following the recommended treatment plan, including all pre- and post-treatment instructions. Consistent Attendance: Missing or rescheduling appointments may impact your results. It is your responsibility to attend all scheduled sessions on time. Lifestyle Choices: Results are heavily influenced by your personal lifestyle choices, including hydration, nutrition, and physical activity. Contour Club is not responsible for outcomes affected by lack of adherence to these factors. 2. Requirements for Best Results Hydration: Drink plenty of water before and after treatments to support your body’s natural processes. Healthy Diet: Maintain a balanced diet to complement your treatments and promote overall wellness. Exercise: Incorporate regular physical activity to enhance and sustain your results. Follow Instructions: Carefully follow all pre- and post-treatment guidelines provided by your technician. 3. Appointment Policies Cancellation and Rescheduling: Appointments must be canceled or rescheduled at least 24 hours in advance to avoid forfeiture of the session. Late Arrivals: Arriving more than 15 minutes late may result in a shortened session or the need to reschedule, at your expense. No-Call, No-Shows: Two no-call, no-show incidents will result in placement on a waiting list. Clients on the waiting list must seek approval before scheduling future appointments. 4. Liability Waiver Acknowledgment of Risk: By receiving treatments at Contour Club, you acknowledge and accept that results may vary based on individual factors, including genetics, lifestyle, and adherence to recommendations. Release of Liability: You agree to release Contour Club, its owners, employees, and contractors from any claims or liabilities related to your treatments. This includes personal injury, dissatisfaction with results, or other outcomes, provided that services were rendered in accordance with industry standards. Dispute Resolution: Should any concerns arise, we are committed to resolving them professionally and collaboratively. By signing our waiver, you confirm your understanding and acceptance of these terms. 5. Refund Policy No Refunds: All sales are final. We do not offer refunds under any circumstances or for any reason, including dissatisfaction with results, changes in personal circumstances, or failure to adhere to the treatment plan.
